You’ll lead through education, quality improvement, and risk management, fostering person-centered care and operational excellence across assigned communities.</p><p><strong>Position Highlights:</strong></p><p><strong>Status:</strong>&nbsp;Full Time</p><p><strong>Schedule:</strong>&nbsp;Typically 40–50 hours per week; on-call for exceptional circumstances only.</p><p><strong>Location: </strong>Hybrid – Pennsylvania (remote with frequent on-site community support)</p><p><strong>Travel:</strong>&nbsp;50%–75%. Regional travel required across NY, NJ, PA, MD, DE, MA, CT, VA, GA.</p><p><strong>Compensation:</strong>&nbsp;$125,000-$135,000 base salary plus bonus&nbsp;</p><p><strong>What You’ll Do:</strong></p><ul><li>Provide guidance to community clinical teams to ensure consistent, high-quality resident care in compliance with regulatory standards and company policies.</li><li>Support and supplement the <strong>Director of Health and Wellness</strong>&nbsp;during vacancies or as assigned for special projects.</li><li>Execute departmental goals, objectives, and policies while monitoring key performance metrics.</li><li>Ensure compliance with all federal, state, and local regulations and organizational standards.</li><li>Track regulatory outcomes, identify trends, and collaborate on corrective or performance improvement plans.</li><li>Participate in the review of incidents, accidents, and clinical risk scenarios to safeguard residents and mitigate liability.</li><li>Lead and educate teams on infection control and prevention programs.</li><li>Guide communities through communicable disease management and reporting to health departments as required.</li><li>Conduct quality rounds and compliance audits; develop and implement corrective action plans as needed.</li><li>Oversee the pre-admission assessment and clinical denial process; review potential move-ins and move-outs.</li><li>Address resident and family complaints and support resolution of grievances.</li><li>Participate in interdisciplinary team meetings focused on quality of care, safety, and satisfaction.</li><li>Collaborate on hiring, onboarding, and professional development of clinical leaders.</li><li>Support community budgeting, staffing plans, and labor management.</li><li>Participate in the annual budget process and monitor clinical financial performance.</li><li>Serve as a champion of company culture, promoting collaboration and accountability.</li></ul><p><strong>Qualifications:</strong></p><ul><li>Minimum of <strong>five (5) years of senior living experience in a leadership role.</strong></li><li>Hold an active <strong>LPN or RN license in Pennsylvania.</strong></li><li>Bring strong <strong>Director of Health &amp; Wellness-level experience.</strong></li><li>Have a deep understanding of <strong>Pennsylvania regulations and operations.</strong></li><li>Be comfortable operating both <strong>hands-on clinically</strong>&nbsp;and <strong>strategically across communities.</strong></li><li>Ability to operate in a <strong>provisional license leadership capacity</strong>&nbsp;for a designated community.</li><li>Strong leadership presence with a <strong>hands-on approach</strong>, including willingness to support floor operations as needed.</li><li>Previous experience as a <strong>Director of Health and Wellness</strong>&nbsp;in an Assisted Living or Memory Care environment.</li><li><strong>BLS Certification</strong>&nbsp;required.</li><li>Experience supporting <strong>multi-state operations</strong>&nbsp;preferred.</li><li>Strong knowledge of assisted living and memory care regulations, clinical practices, and quality standards.</li><li>Ability to identify and respond to high-risk resident concerns with timely, appropriate interventions.</li><li>Proficiency in <strong>Microsoft Office</strong>&nbsp;and ability to learn new software systems.</li><li>Exceptional organizational and time management skills; ability to manage multiple priorities effectively.</li><li>Strong interpersonal and communication skills; able to interact courteously and professionally with residents, families, and team members.</li><li>Demonstrated leadership, problem-solving, and decision-making skills.</li><li>Competence in risk management, staff education, and compliance auditing.</li><li>Must demonstrate an interest in serving an aging adult population.</li><li>Ability to travel frequently and work a flexible schedule as needed.</li></ul><p><strong>Why Join Us</strong></p><p><strong>Discovery Senior Living Earned Coveted Great Place to Work Certification for 2025-2026!</strong></p><p>A large part of what makes Discovery different from other companies in this industry is our willingness to embrace change.
